What is Shared Care?
The Shared Maternity Care Program enables the woman’s birthing hospital of choice and a community midwife share a woman's pregnancy care.
Women who undertake shared maternity care will see their shared maternity care Midwife for most of their antenatal care and also have appointments at the hospital in early and late pregnancy.
Research shows that having a known care provider during pregnancy results in less hospital admissions, less interventions in labour and birth, higher breastfeeding rates and women being more satisfied with their childbearing experience.
When I'm your
share cared Midwife
HOW IT WORKS
✅ YOU obtain a referral from your 👩⚕️ GP to Kate at GROW.BIRTH.MOTHER. (once you're pregnant of course 😜) and to your 🏥 hospital of choice to birth at requesting shared care.
✅ YOU have a booking-in appointment 📆 with myself & your hospital of choice at sometime between 10-16 weeks.
✅ I for the remainder of your 🤰pregnancy & 🤱postpartum can organise all your scans & blood test that you may wish to have performed.
✅ You & I have 📆 appointments together at roughly 16 weeks, 20 weeks, 31 weeks, 34 weeks, 38 weeks 39 weeks and 40 weeks.
✅ You will have check in 📆 appointments at your 🏥hospital of choice at 28 weeks, 36 weeks & 41 weeks (if needed).
✅ YOU rock your birth at your 🏥 hospital of choice.....with me on speed dial if you have any questions or topics you want to pick my brain 🧠 on in early labour.
✅ YOU stay at the 🏥 hospital as little or as long as you feel comfortable, and then I can support your 🤱postpartum care for up to 6 weeks postpartum.
